Are you or your child consistently forgetful or disorganized? Do you have trouble getting started on things? Do you experience life more intensely than others?

This quiz is designed to identify common ADHD symptoms in yourself or your child. Answering “Yes” to a significant number of these questions may indicate further exploration may be necessary.

Note: Only a trained and licensed professional has the ability to accurately diagnose ADHD. There is no single test for diagnosing ADHD. A comprehensive evaluation is necessary, by a school psychologist, social worker, nurse practitioner, psychologist, psychiatrist, neurologist, or pediatrician.

Avoid starting large projects that require a lot of parts and organization.

Have a tough time finishing projects once the interesting parts are done.

Tune out and get distracted when people are talking to me.

Tend to make mistakes on details when projects aren’t interesting.

Have a hard time remembering what I need to do.

Often late for appointments and meetings.

Forget where put things, and forget to bring things like homework, lunch, or documents.

Have trouble sitting when expected, such as in a meeting or watching TV with the family.

Talk a lot; do not wait turn in social situations.

Interrupt boss/parents when they’re busy.

Tend to get in a zone of focus without taking a break, or to eat or sleep.

Get frustrated and restless easily when things happen slowly.

Need a lot of stimulation from things like video games, extreme sports, buying new things and risky activities.

Have trouble winding down.

Have challenges getting to bed, staying asleep and/or waking up.

Often not aware when annoying people.

Have biological relatives with ADHD, mood disorders, or substance abuse problems.
